Job Description:

• Grand Traverse Pavilions is seeking a motivated, full time Certified Nurse Aide to join our team. Variety of shifts and hours available. 

• The starting wage for this position is $22.22/hour.

• $2.00/hour for working afternoon and midnight shifts.

• $1.00/hour for working Saturday and Sunday shifts.

• $2.35/hour Essential Worker Bonus.

Tasks:

• Daily checks resident care plan for those residents assigned so that Nurse Aide is aware of treatments and special nursing measures required for the shift.

• Assists residents in personal hygiene;  e.g., a.m./p.m. care, peri-care,  bathing, skin care, shampooing and grooming of hair, mouth care, brushing teeth, shaving, nail care, dressing and undressing. 

• Answers call lights promptly and attends to resident needs.

• Prepares resident for meals, transports resident to dining room or places resident in appropriate position in their room or bed for the meal. 

• Serves food trays, feeds resident requiring help, record dietary intake, collect finished trays for return to Dietary and passes nourishments between meals.

• Checks incontinent residents no less often than every two (2) hours, or more often as required by care plan.  If dry, takes resident to the bathroom or if

incontinent, washes and dries resident.  Promptly changes resident's clothing or bedding when it becomes wet or soiled with urine, feces or other secretions or excretions.

• Provides restorative nursing care to include range of motion, positioning and body alignment, preventive skin care, transfer and ambulation training, bowel and bladder training and assists with activities of daily living.

• Documents on residents care record daily.

• Encourage independence, socialization and participation in activities.  Prepare residents for departmental activities or hospital out-patient services and transport residents to and from activities and appointments.

• Assists with the care of the critically ill.

• Notifies charge nurse of any change in resident need or condition, injury or potentially unsafe activity or event.

• Performs all other duties as assigned.

Tasks:

• Assist residents with personal care including grooming/hygiene, dressing/undressing, shower, bathing/whirlpools, toileting and incontinence management. 

• Distribute medications and provide treatments to residents as indicated in plan of care. 

• Provide stand-by assist for resident transfers (on/off toilet, in/out bed, in/out chair, etc.) 

• Communicate residents’ needs and health status to Supervisor verbally and in writing.

• Responsible for organizing own work effectively, assisting the Nurse Aides to organize their work, and supervising the quality of nursing care on an assigned shift.

• Make resident rounds initially and regularly throughout the shift monitoring resident needs, change in condition, safety, proper functioning of equipment, employee performance, and follow through of physician orders. 

• Provide resident care as assigned.

• Ensure that medical record documentation is complete by the end of the shift per procedure.

• Assist in maintaining current care plans according to condition changes and advise supervisor accordingly.

• Ensure adequate nutritional intake by residents for assigned shift.  Ensure that resident nourishments are offered/provided timely, and recorded.

• Notify responsible party of changes in resident’s plan of care.

• Administer and document medications, controlled drug accountability, ordering medications, and returning unused medications to pharmacy.

• Direct and/or perform and record procedures, treatments and response and complete charging procedures as required. Administer and document treatments per policy, and complete associated documents accordingly.

• Monitor IV infusions for proper flow rate, potency, etc. Report problems to supervisor.

• Recognize and take appropriate action to eliminate or minimize a potential or actual hazard to a resident and to initiate emergency measures as appropriate.

• Report suspected infections to the Infection Control Coordinator.

• Actively participates in quality assurance monitoring and corrective actions.

Tasks:

• Work cooperatively and respectfully to ensure smooth operation of the Child Care Center.

• Assist with the general operations of the Child Care Center to include, but not limited to, opening and closing of the Center as assigned.

• Plan and implement individualized, play-based, developmentally appropriate activities for an assigned group that are balanced within child developmental principles.  To include but not limited to indoor and outdoor play; activities coordinated to theme of the week within program; activities that challenge children’s physical and cognitive abilities, encourage children to learn and explore and foster self-esteem.

• Supervise daily activities of children to include but not limited to meals, naps, toileting, play and developmental programming.

• Advise management of inventory needs or equipment repair needs.

• Maintain program equipment and supplies in an orderly, clean fashion.  This includes cleaning toys, equipment, tables, etc. suitable to meet Grand Traverse Pavilions standards.

• Report any concerns regarding a child(ren) to the Child Care Facilitator in a timely manner.

• Responsible for the health, welfare and safety of children in designated group.

• Assist with dressing and changing diapers as necessary.

• Escort and assist all children to intergenerational planned activities and during any other room changes.

• Abide by applicable policies and procedures.

• Complete paperwork related to children’s progress, attendance, behavior, etc. as required.

• Substitute for other staff positions in Child Care as requested in order to meet the immediate needs of the Center.

• Perform all other duties as assigned.
